Supersub Jay Rodriguez claimed his second dramatic knockout winner of the season to clinch a last-gasp win for Burnley's marathon men.
Rodriguez pinched the ball off Damion Stewart's toe and poked a shot past keeper Radek Cerny in the 120th minute with a piece of trademark cool finishing.
The local lad had scored the late winner against Fulham when he came off the bench in the Carling Cup at the start of boss Owen Coyle's run of stunning success in cup contests in September.
Rodriguez, 19, was needed as Burnley ran out of players in a gruelling replay - with the victory bringing yet another shot at glory away to Premier League West Brom in the next round.
It looked grim for Burnley in the 54th minute when QPR's Italian rookie hitman Samuel Di Carmine slotted home.
But six minutes later big Scot Steven Thompson scrambled an equaliser with a shot off the underside of the bar.
